When our services are chosen, individualized care plans are developed, managed and utilized to treat each patient's and family's special needs. In fact, our patients are involved with the plans that we design with them. The patient's doctor, along with the Valor team, the patient and Medical Director, will determine the initial course of treatment and review the plan regularly in order to assure that the patient's changing needs are met.
In addition to regular home visits by a nurse, the following services for the hospice diagnosis are provided:
