Valor HospiceCare offers as part of the Medicare Hospice Benefit, palliative medicine evaluaton services for all patients with serious illnesses. This is available 24/7 to assist community physicians, healthcare professionals and the community for patients in physician practices, private homes, adult care homes, group homes, assisted living facilities, skilled nursing facilities and hospitals. Upon request, a highly trained hospice physician, nurse practitioner or nurse will make a patient visit to assess complex symptoms and discuss findings and suggestions with the requesting physician. We assess the function and safety of your patients and their caregivers in their own home environment, including adherence to your plan of care, medication regimens, mobility and falls risk, among other areas. Our goal is to complement your plan of care by ensuring that your patients with serious chronic illnesses and/or physical/mental limitations have access to resources and conditions needed for them to succeed at home.
These evaluations are available for all patients, regardless of age, and are not limited to those patients considered hospice-appropriate, including:

Upon request, a highly trained hospice physician or nurse practitioner will make a patient visit to assess complex symptoms and discuss findings and suggestions with the patient or the requesting physician or patient facility. We ensure that hospice patients with serious chronic illnesses and/or physical/mental limitations have access to resources and conditions needed for them to succeed at home.

Please note that eligibility in the palliative consultation and primary home care programs depends on patient insurance carrier benefits from Medicare Part B, Medicaid/AHCCCS, Managed Care and Private Insurance Providers, and the availability of contracted physicians and other healthcare providers.
